Authorization and signature
Understand that we will not perform any repair work until we have provided you with an explanation of what
repairs are needed and what the cost of those repairs will be. We want you to be informed and have a clear
understanding of what repairs we will be performing. We do have a minimum of 1 hour for diagnostics. If more
time is needed, we will notify you first. Understand we are not interested in making a quick dollar off of
anybody and we desire to be your repair shop of choice when it comes to meeting your repair needs. We will not
pressure you to spend unnecessary dollars and can only inform you of needed or recommended services.

Repair Authorization
I hereby authorize Haggai Truck Center, LLC including its officers, agents and employees, to estimate and
repair my vehicle. I grant Haggai Truck Center, LLC permission to operate the vehicle for the purposes of
diagnosis, testing and inspection. An expressed mechanic's lien is acknowledged for amount of repairs in
addition to releasing the business from liability for loss or damage to vehicle or articles left in vehicles
in case of fire, theft, or other cause beyond the control of Haggai Truck Center LLC. I understand daily
storage at the rate of $35 per day will be applied to my bill after 3 days of my unit being finished.
